The Community Health Partnership Fund is a collaborative initiative by Confluence Health and Wenatchee Valley Medical Group dedicated to transforming regional health. The Fund’s purpose is to uplift community health and well-being through impactful projects and initiatives that inspire positive change and foster a healthier future for all.
The Community Health Partnership Fund awards up to $500,000 annually to programs that enhance health and wellness and address social determinants of health in Chelan, Douglas, Grant, and Okanogan counties.
Applicants must be a 501(c)(3) nonprofit or government entity (e.g., schools, emergency response) serving one of these counties or operate under a 501(c)(3) fiscal sponsor. The Community Health Partnership Fund may consider applications from faith-based organizations if programs and services are open and accessible to all, regardless of faith or belief, separate from faith-based activities or instruction, and without requirements for participation in faith-based activities to receive services. Please submit only one grant request per organization. The maximum request is $50,000, but the median award in 2025 was $7,000. Funds may only be used for direct program or capital support.*
This fund is managed by the Confluence Health Foundation, ensuring that every grant supports meaningful and sustainable improvements in our community.
Projects that directly support comprehensive health and wellness access and activities.
Initiatives that address social determinants of health, such as food and housing security, early education, economic stability, and social support systems.
Applications are accepted through our grant portal beginning June 1 and applications will be accepted through July 31.
